Do you need support to attend your healthcare appointments?
Through our Core20PLUS5 Project we offer appointment support to those 18+, struggling to attend healthcare appointments within the Wakefield District.
There are many reasons as to why someone may benefit from this support, such as mental health issues, learning disabilities, memory issues, a lack of confidence using public transport or difficulty navigating a hospital. We want to support people to overcome any barriers they may be facing in attending their healthcare appointments, to improve their overall health and wellbeing.
Once we receive a referral, Live Well Wakefield will contact the individual to discuss what level of support they need and any preferences they may have.
If we are able to support, we will then allocate the inidivdual to one of our partner Community Anchors who will assign a Peer Support Volunteer and support them through the logistics and needs for the appointment. A Social Prescribing conversation will also be offered, to help address any non-clinical needs, creating links to local support options to improve independence and wellbeing.
Our support is free, and all our staff and volunteers are DBS checked.
Criteria
- Aged 18+
- Lives in Wakefield / or registered with a Wakefield District GP
- Able to self-care whilst out/after appointment
- Has a healthcare appointment booked, that is taking place within the Wakefield district
- Able to self-fund own travel
