Supporting people to improve their health and wellbeing

Through social prescribing and self-management support, we help individuals to explore their social, emotional and practical needs, and to understand how these needs impact our overall wellbeing. We work together to identify what matters most and create connections to local services, community groups and personalised advice.

We support people who are 18+ and live in the Wakefield district, or are registered with a Wakefield district GP.
