We are a dedicated social prescribing and self-management service for adults in the Wakefield district. Commissioned by Public Health at Wakefield Council and local Primary Care Networks, the service is delivered as a partnership between South West Yorkshire Partnership NHS Foundation Trust and Nova Wakefield District, and aims to address non-medical, social needs through personalised support and community connections.
Although the service has been delivered in its current form since October 2023, our roots in serving the local community extend back to 2017, working consistently to empower individuals by connecting them with local resources and support systems, helping them manage their wellbeing and improve their quality of life through our core services and several successful projects.
Our team is made up of experienced individuals with an array of skills and expertise, with extensive knowledge of support available across the Wakefield district, enabling us to offer individuals tailored support to best address their needs.
Our work has been recognised in the feedback we receive from those who access our service, and we have been successful in winning or reaching the final of several awards at a national and local level, including:

- Social Prescribing Link Worker of the Year – Winner, 2022 Social Prescribing Network Awards

We recognise that many factors such as loneliness, finances, employment, or isolation can affect our wellbeing. We work with individuals to identify the challenges they face, support them to set goals, and connect them with the most appropriate local support to address their non-medical needs.
