Social Prescribing support

Social prescribing is a personalised approach to improving health and wellbeing. It connects individuals with local services, activities, and support networks to address non-medical needs and health related barriers that may be affecting their ability to consider employment as a positive next step in their future.

Our service offers dedicated one-to-one support, helping individuals build resilience, improve wellbeing, and take meaningful steps towards their long-term goals.

  • Mental health and emotional wellbeing

  • Personal safety
  • Education and training opportunities
  • Lifestyle and wellbeing advice

 

What to expect

Following a referral, a member of our team will contact the individual to explore their needs and priorities. Support is delivered in a way that suits them best:

  • Telephone consultation
  • Face-to-face meetings in community settings
  • Appointments at GP surgeries
  • Home visits

 

Personalised care and support planning

Using a NICE-endorsed social prescribing tool, we co-create a Personalised Care and Support Plan that includes:

·       Identification of individual needs, preferences, and future employment-related goals

·       A structured action plan to address barriers to wellbeing and readiness to considering employment as a positive step in the future

·       Referrals and connections to relevant local services, including voluntary, community, and statutory organisations

Our aim is to facilitate access to the right support, enabling individuals to improve their overall wellbeing and move closer to employment readiness.

 

 

Self-management workshops

In addition to one-to-one support, individuals can access a range of self-management workshops designed to build confidence, develop skills, and support long-term wellbeing. Visit our self-management support page for details on available topics and booking information.
