I want support to self-manage my long term condition

We understand the uncertainties of living with or caring for a person with a long-term condition or self-managing aspects of life which are sometimes difficult. We practice every day the tools and techniques that you can learn by attending any of the services that we run. We want you to develop and improve the quality of your life.

The Live Well team can guide you through the support options that are available to help you manage a long-term health condition. This could be your own long-term health condition or that of someone you care for.

There are a range of options available to help meet your individual needs:

Workshops

We offer a variety of self-management workshops, which include titles such as  "Managing Fatigue with Health Conditions" and "Coping with Pain".

Each workshop runs for 1 hour and these are available for you to attend online from your own personal space without the need to travel, using the free app Micorosft Teams using either a smartphone, tablet device or laptop.

Volunteering

All our programmes are facilitated by staff and dedicated volunteers. Each one will have received training for their role and will be sharing  the skills that they have learned to help others like you.

We also have other volunteering opportunities with our service, so please get in touch for more information about volunteering by calling our office on 01924 255363.

Why would someone choose this service?

In the last 3 years, 100% of those who attend a course or workshop say they would recommend attending to a friend or family member.

The service is completely free and confidential.

Our information doesn't affect any treatment you may be already receiving as it compliments other health care programmes.
