We understand the uncertainties of living with or caring for a person with a long-term condition or self-managing aspects of life which are sometimes difficult. We practice every day the tools and techniques that you can learn by attending any of the services that we run. We want you to develop and improve the quality of your life.
The Live Well team can guide you through the support options that are available to help you manage a long-term health condition. This could be your own long-term health condition or that of someone you care for.
There are a range of options available to help meet your individual needs:
Self-Management programmes
Our self-management service runs 2 different 6-week programmes (each session lasts 2.5 hours) for anyone who has any long-term health condition which affects their health and wellbeing. There are both online options, where you can join the group sessions from your own home (using the free app Microsoft Teams) or there are face-to-face groups, taking place in community buildings, across the Wakefield district.
All are delivered as group sessions, once a week over 6 weeks and all are led by people who are also living with one or more health conditions themselves. They have also taken part in a programme, finding it helpful in managing their own wellbeing and have then undergoine training to deliver the programmes for others like you, living with the impact of long-term health conditions.
We are also supported by our volunteer team, who bring their own valuable lived experience to the programmes, delivering the sessions alongside our staff members. All our volunteers have undergone training to support them in their role.

Workshops
We offer a variety of self-management workshops, which include titles such as "Feeling Positive, Busting Stress" and "Coping with Pain".
Each workshop runs for 1 hour and these are available for you to attend online from your own personal space without the need to travel, using the free app Micorosft Teams using either a smartphone, tablet device or laptop.
Volunteering
All our programmes are facilitated by staff and dedicated volunteers. Each one will have received training for their role and will be sharing the skills that they have learned to help others like you.
